Mastopexy or Breast Lift
is performed for women with aging or sagging breasts for purposes of
rejuvenation or enhancement. Augmentation Mammoplasty, popularly known as
Breast Augmentation, utilizes an implant (saline or silicon) in order to
enhance the breasts. With an over-sized breasts that are severely droopy,
usually causing back pains, Breast Reduction (Reduction Mammoplasty) is
recommended.

How is the breast lift done?
Lifting the breast is indicated for those with droopy breasts. The
different incisions are designed to remove the excess skin, thereby
lifting the breast upon closure of the skin. Specific incision-designs are
based on the severity of the breast ptosis – sagging of the breast. Other
incisions leave an apparent scar, especially for the dark skinned
individual with a significant breast ptosis. Mildly droopy breasts can be
lifted by using an implant, thereby achieving enhancement and augmentation
as well.
Where is the implant placed when doing Breast Augmentation?
When the implant is placed within the breast tissue and above the muscle (pectoralis
major) the procedure is called a subglandular augmentation. Remember that
the breast rests on the pectoralis major muscle. The other approach places
the implant below the pectoralis major, this is called the subpectoral
augmentation. When the implant is hidden underneath the muscle, it is not
apparent when holding the breast. The latter approach is preferred.
Where will the scar be?
It depends on where the implant is inserted. The implant can be inserted
through an incision around the areola, or through an incision over the
skin fold between the breast and the chest wall; however, the scar can be
better concealed when the implant is inserted through the axilla (armpit).
Remember, scars are more obvious for those with a darker skin color.
Can fat tissue be used instead of the implant?
The use of transplanted fat tissue, as free fat grafts is disappointing
due to an unpredictable rate of resorption. The transferred fats are
eventually absorbed by the body.
Why was there a moratorium on the use of silicon implant in the United
States?
In 1992 there were laboratory studies that revealed breakdown on the
implant with formation of toxic byproducts. There was a possible link
between the use of silicon implant and lupus, rheumatoid arthritis, and
mixed connective tissue disease. Hence, the Americans shifted to the use
of saline implants.
What is the status of the use silicon implants now?
Since 1992, many reports have appeared to refute the claims mentioned
above. Thus, when the Americans stopped using silicon for breast
augmentation, in Europe and Asia, silicon implants were continually being
used. Finally in 1999, the Institute of Medicine (IOM) of the National
Academy of Sciences reported that there is no association between silicon
gel implants and cancer, immunologic disease, or other systemic diseases;
and they pose no risk for breast feeding or to unborn infants.
What are the side effects of silicon injection?
You must avoid the use of injected silicon over the nose, breast, or any
part of the body. With this method, the silicon becomes admixed with the
tissues causing redness and eventually granuloma formation - multiple
lumps. The problem is in the removal of these lumps. The breast tissue can
not be separated from the lumps of silicon. Removal of the silicon often
ends up with a mastectomy or the surgical removal of the whole breast. The
use of injected silicon has been abandoned since 1975.
What is capsular contracture?
There is a capsule that eventually forms around the implant. This is
exhibited as breast firmness. When firmness becomes symptomatic – painful,
hard, with visible distortion - it is termed capsular contracture. This is
the most common problem encountered after breast augmentation using a
silicon implant.
How is breast reduction being done?
Reducing the size of the breast is almost similar in incision-design when
lifting it (1st question). However, with the excision of excess skin comes
the removal of breast tissues. Again, depending on the severity of the
breast size and ptosis, the appropriate incision-design may leave an
apparent scar. A more obvious scar is a trade off for a significant breast
reduction.
